Email guidelines
  1. Please understand that emergency needs cannot be met by email and that email may not be checked on a regular basis. Urgencies should be referred to the office or the 24-hour answering service. Emergencies should be handled by calling "911"
  2. If the problem that you plan to email is complicated or long, it would be better for you to schedule an appointment with one of our physicians. If you email an extremely complex problem, we may email you back and ask you to schedule an appointment. Security of email messages is limited by your server, computer, software, browser, and access to your system.
  3. Security is appropriately controlled on our website and email system. Email is not a substitute for an office visit and/or physical examination. It is only a convenient means of introduction or communication to handle simple needs, questions, or drawing attention to various aspects of your care.
  4. Copies of your email correspondence will be kept in your chart.
  5. If you abuse your email privileges, such electronic interaction may be terminated at any time, by email filters and your messages will not be read.

Insurance Information
We accept most major insurance plans.

Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care.

We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.

International Patients
Please call Veronica Lubians at (954) 907-9445 to schedule an appointment or a procedure. Veronica can assist in making the consultation process efficient and comfortable.

Fees & Payments
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard, Visa, Discover, and American Express for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.

Prescriptions & Renewals
Please request all prescriptions and authorizations during our regular office hours, when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances.

For Your Appointment
If you are a new patient, please arrive to your appointment early in order to complete the registration process. In addition, please bring the following:

> Driver's License or State ID
> Referral (If Required by Insurer)
> Insurance Information
> Copies of Operation Records, Medical Records, X-rays, MRIs, and CT Scans from Prior Doctor Visits
> A List of Current Medications and Any Known Allergies

Remember To Bring
We try to make every part of our process as simple as pain-free as possible. It’s our philosophy that patients truly come first.

1. Your ID.
A valid form of photo identification is a must on your first visit.

2. MRI CDs, X-ray Images, CT Scans.
These images and/or films are necessary to make any proper diagnose and treatment plan.

3. Patient Paperwork
We have provided paperwork online to save you time. You have the option to download, print and complete prior to your appointment.

Medical Forms
Provided on our website are various forms that require completion prior to your visit and/or surgery. Please download the appropriate form as directed by your physician and/or staff by clicking on the name of the form. If you cannot access the form, please click on the Adobe icon. This will allow you to download this software, providing access to the forms.

Please bring the completed form(s) with you to your scheduled visit.
This will help expedite the registration process.
